Prior to your first session, there are several forms to complete that will help me with your inital assessment. ü Any lab work from your medical record. ü A list of medications and/or the bottles. ü A list of any supplements and/or the bottles. Completing this information prior to your first session lets us spend more time focused on your assessment, your nutritional needs, and your health. In order for your time with Cheryl to be spent effectively, it is important that you prepare a food diary a few days in advance. Why keep a Food Diary? A minimum of 24 hours notice is required for cancellation or rescheduling of all appointments.

Once we have scheduled an appointment, I will email or mail a Nutrition Packet / Questionnaire and a Physician Referral Form to you.
Please complete the Nutrition Packet / Questionnaire Forms, Physician Referral Form and bring all the required forms with you to your appointment, along with:
What to expect during your appointment
At your appointment, Cheryl will obtain a diet history and assist you in making long term changes to your eating pattern and nutritional intake and provide strategies for overcoming obstacles. As your nutritionist, she will monitor your ongoing dietary and nutritional needs and will continue to follow-up your progress through future appointments to provide an opportunity for continued learning.
Preparing for your appointment
A Food Diary is a very effective tool that can help you determine if your eating habits are healthy. Please bring your completed food diary with you for our nutrition counseling session.
